PACE Care
The Program of  All-Inclusive
Care for the Elderly

"PACE can help - with an option to nursing home care for your loved one."

Elderhaus is entering its fourth year serving PACE participants. Elderhaus became the first PACE site in North Carolina in 2008! PACE stands for Program of All-Inclusive Care of the Elderly.

The PACE model was developed to address the needs of elderly and frail elderly individuals who want to remain in the community and receive home and community-based services rather than receive care in a nursing home. Once enrolled, the PACE Program provides full medical care, social services, and personal care services required by the individual.To be considered for PACE, you must:

  Live in New Hanover or Northern Brunswick County
  Be over 55
  Determined by the state to be eligible for nursing home level of care
  Be able to live safely in the community with a care-giver.

If you are eligible for Medicaid, and depending on income, PACE services are offered free of charge.  If your income is over the threshold determined by Medicaid, you may still be eligible to participate by paying a premium.

lderhaus PACE provides:

  Medical Care
  Primary and Specialty Care
  Emergency and Urgent Care
  Prescription medications, including home delivery
  Dentistry, Podiatry and Optometry
  Outpatient labs and X-rays
  Long-term care
  All Transportation to Elderhaus and doctor visits


Social Work
Ongoing supportive case management services.

On-going Rehabilitative Therapies
Physical, occupational and speech therapy, nutrition counseling
Medical equipment, such as walkers and wheelchairs

In-Home Care Services
Personal care and light housekeeping

 Day Care Activities
Recreational therapy programs such as art, music, exercise and field trips.
Lunch is provided.

The social worker will be able to help you step through the process of determining eligibility for Medicaid and PACE.

Elderhaus PACE operates under a contract with the North Carolina Division of Medical Assistance and the Centers for Medicare and Medicaid Services. Elderhaus PACE participants must receive all covered services, except emergency services, through Elderhaus PACE staff or its contracted network of providers and facilities. To be covered, all non-emergency services must be authorized by Elderhaus PACE. Elderhaus PACE participants who obtain services without Elderhaus PACE authorization may be personally and fully liable for the costs of the unauthorized services.
