Program of All-Inclusive
Care for the Elderly "PACE can help - with an option to
nursing home care for your loved one."
Elderhaus is entering its
serving PACE participants. Elderhaus became the first PACE site in North
Carolina in 2008! PACE stands for
Program of All-Inclusive Care of the Elderly.
The PACE model was developed
to address the needs of elderly and frail elderly individuals who want to remain in the
community and receive home and community-based services rather than receive care in a
nursing home. Once enrolled, the PACE Program provides full medical care, social
services, and personal care services required by the individual. To be considered for PACE, you must:
Live in New Hanover or Northern Brunswick County
Be over 55
Determined by the state to be eligible for nursing home level of care
Be able to live safely in the community with a care-giver.
If you are eligible for Medicaid, and
depending on income, PACE services are offered free of charge. If your
income is over the threshold determined by Medicaid, you may still be
eligible to participate by paying a premium.
