PACE is a Program of All-inclusive Care for the Elderly (PACE). PACE is based
upon the successful long-term care model in San Francisco known as On Lok. This
program was developed as a special health plan for senior citizens. Older
people often have medical problems that last for long periods of time. This
health plan will arrange for doctors, nurses or other specially trained people
to help with these problems. It also gives Elderhaus PACE participants a place
to go to receive this care.
Elderhaus PACE is a complete health care program that offers a more personal way
of getting health care. All of us at Elderhaus PACE want to get to know you and
our staff will work with you and your family to provide the care that you need.
Elderhaus PACE operates
under a contract with the North Carolina Division of Medical Assistance and
the Centers for Medicare and Medicaid Services. Elderhaus PACE participants
must receive all covered services, except emergency services, through
Elderhaus PACE staff or its contracted network of providers and facilities.
To be covered, all non-emergency services must be authorized by Elderhaus
PACE. Elderhaus PACE participants who obtain services without Elderhaus PACE
authorization may be personally and fully liable for the costs of the
unauthorized services.
