Adult Day Care provides an
organized program of services during the day, in a community group setting, for
the purpose of supporting personal independence and promoting social, physical,
and emotional well-being of adults.
Adult Day Health provides the
same organized program as outlined above but requires the services of a nurse
for monitoring a medical condition. Adult Day Health participants usually
require more assistance with activities of daily living than Adult Day Care
participants.
Both Adult Day Care and Adult
Day Health participants may require administration of medications, special
feedings, or provision of other treatments as ordered by a physician. Each Adult
Day Health participant has a plan of health designed and monitored by our
registered nurse.
All participants have
individual service plans that outline their individual needs and interests.
Program goals are designed to meet these individual needs.
Elderhaus Admission Process
1. The applicant's primary care physician must complete a Medical
Examination Form.
2.
An enrollment conference will be
held with the applicant and caregiver to discuss policies, complete paper work,
and obtain information about the applicant that will be helpful in designing an
individual service plan..
3.
The applicant and caregiver visit Elderhaus, have a tour and meet staff.

Daily Fees
The costs for Adult Day Care and Adult Day
Health are $47.00 and $53.00 respectively. There are monies available
for assisting with covering the costs of day care services. Costs should
never prohibit anyone from inquiring about the program.
